How to compare hair transplant quotes (and spot a hair mill)

A quote is more than a number. To compare two quotes honestly you need to turn them into the same units, list what's missing, and find out who will actually do the surgery. Here's a checklist built from the 150 prices in our database and the warnings published by surgeons' societies.

Step 1: Get the same five facts from every clinic

  1. Total price, in writing, with the currency, and whether tax and fees are included.
  2. Graft count, or the maximum the price covers. A package with no graft number can't be compared.
  3. Technique (FUE, DHI, FUT, robotic), and the price of the alternatives at the same graft count.
  4. Inclusions: hotel nights, transfers, PRP, medication, blood tests, follow-up.
  5. The named doctor responsible for your care, and which steps they perform personally.

Only 25 of the 150 prices in our database state their inclusions, and many packages give no graft cap. Expect to ask for most of this yourself.

Step 2: Turn every quote into price per graft

Divide the total by the grafts it covers. Then you can compare it with real medians: $1.72 for Turkey, $1.32 for Mexico, $1.47 for Colombia, $3.85 for the UK, and $6.50 for the US. Those are median starting prices from our country ranking, so a quote that comes in somewhat above them is normal. For example, a patient-reported Turkish quote at HLC in Ankara was €7,290 for 2,700 grafts, which is €2.70 per graft. A US patient was quoted $16,000 for 2,000 grafts, or $8 per graft.

Step 3: Strip out travel and compare the surgery

If one package includes three hotel nights and another doesn't, the first will look more expensive. Price the hotel and transfers yourself, subtract them, and compare what's left. DK Klinik's own price list shows a three-night hotel adding €200 to the package. Our package guide covers each line item.

Step 4: Check the red flags

A "hair mill" is the informal name for a high-volume clinic where unlicensed technicians perform surgical steps and the advertised doctor has little or no involvement. The International Society of Hair Restoration Surgery (ISHRS) runs a public campaign about it, called Fight the FIGHT (ISHRS Fight the FIGHT campaign). Its published red flags (ISHRS Fight the FIGHT: hair transplant black market red flags) include:

The ISHRS also points out that a doctor's brief appearance or hairline sketch doesn't answer the real question. It suggests asking who will diagnose you, design the hairline, give anaesthesia, make the incisions and perform the extractions, which parts technicians handle, and who manages your care if there's a complication.

Step 5: Price-based warning signs

Step 6: Verify, then decide

A simple comparison worksheet

Put every quote into one table before you decide. Use one row per clinic and these columns:

ColumnWhat to write
Clinic and named doctorThe person responsible for your surgery, not just the brand
Total price and currencyAs written on the quote, then converted to your currency on the same day for every clinic
Graft count or capThe number the price covers, and the price for extra grafts
Price per graftTotal divided by grafts
Travel items includedHotel nights, transfers, interpreter, and what you'd pay for them yourself
Not includedTax, fees, medication, follow-up, card surcharges
Who does each stepDiagnosis, hairline, anaesthesia, incisions, extraction, implantation
Deposit and refund termsHow much, when, and what happens if you cancel

A clinic that won't help you fill in a row is telling you something. Leave the cell blank rather than guessing, the same rule we follow in our own database.

A note on reviews and "featured" clinics

Reviews can be bought, and some directories rank clinics by who pays. On HairLossCosts, paid placements will always carry a visible "Featured · paid" label, payment never changes price data or ranking, and clinics can't pay to remove patient quotes (see our disclosure). Wherever you research, ask how rankings are decided.
